For most of medical history, it was all paternalism. The doctor would say, you need to do X and then the patient would do X. Now there is an emphasis on what they call shared decision-making. Most common thing that people have to get at least forms filled out for informed consent has to do with giving blood. But ultimately I haven't seen anyone saying, no, I don't want the blood transfusion.
