Dr. Tripty, can you walk us through what findings would trigger your suspicion for PAPVC and what diagnostic strategies you would use to further evaluate these situations? Dr. Harris: On EKG, RV enlargement without left heart dysfunction should raise suspicion for anomalous pulmonary venous connection. Other hints can include the presence of a sinus venosis ASD, a secundum ASD, or RV enlargement that's significantly large for a small ASD or PFO. A trans esophageal echocardiogram can instead be used to identify the site and drainage of the pulmonary veins.
