It was inspired by his time as a sex worker in the 70s. He wanted to do a book about sexuality, like about sexuality and good and evil. And then it's also inspired, I guess, by S&M clubs where people would just get piercings for the fun of it. Which I think is why the guy has all the pins in his face. It's the same nerves, it's just what you do to him.
