Social prescribing is a process through which health professionals refer their patients out into the community. A person called a link worker or a social prescriber connects that person with community resources based on their individual needs. The idea is that it can address the social determinants of health, which probably account for about 20% of presentations to general practice. And once you get prescribed a link worker, that link worker helps you to get involved in something in the community, a group or an activity.
