If your patient is breathing through a trache, they need to have some mechanism to humidify their secretions. Usually, I just use albuterol for my nebulizers. Hypertonic saline draws water into the mucus, which thins out those thick, subrinsicretions and allows your patient to clear them. What about NABs? Yeah, with NABs, simplicity is key. Don't try to reinvent the wheel.
