Is There an Explosion on the Airplane?
Eye witness accounts of what they see can be very unreliable, especially when it comes to planes. There was no explosion on the plane, but there were multiple people on the ground who said they saw the plane explode in the air. It's really strange how your mind can play tricks on you and make you think you see things that weren't really there. And then the last cause is insufficient corrective action from the first officer in order to cover the consequences of the first factors.
