Nitro and Diastolic Blood Pressure

A lot of the guidelines use a systolic blood pressure as the cutoff for, whether you're gonna give nitro or not. The diastolic is really what's profusing your coronary arteries and so like if I have a blood pressure of 120, but the diet, let's say it's 120 over 50 or 60, that doesn't make sense to give nitro rate. We did a study here at Hanathan in 2008 where 87 patients who had a STEMI got nitro after getting their first EKG by the time of arrival, only three patients had resolution of their ST elevation on the ECG. So that's evidence that nitro glycerin has no effect
