The same set of symptoms has very different ramifications for different clients. It's not just a symptom, but how it's practically affecting that person in the real world. The next piece is important to document relates to the client's response. Do they appear to be responding positively to the therapeutic encounter? Are they open to your relapse prevention interventions?
