The patient with IPF has an acute worsening usually defined as less than 30 days of increasing respiratory symptoms and new bilateral ground glass capacities on the CT scan. And if it remains a acute exacerbation by definition, excluding another cause that we can treat, then we generally give corticosteroids. These patients histopathologically, when biopsies were done, we certainly don't do that generally anymore, had diffuse alveolar damage or organizing pneumonia. But the patient with organizing pneumonia are probably the patients in whom the corticosteroid make a big difference.
