Not all small kidneys are due to rin vascular disease especially for younger patients it's fairly common that they've either got a congenital um urologic abnormality maybe they've had kidney infections in childhood. The key is you're not necessarily trying to salvage one kidney you're trying to salvage of probably other kidney. A good screening test for that group might be in a abr so an ankle brachial blood pressure or high cardiovascular risk patient again i treat medically and then pursue the renovascular workup if they're not behaving like they should or there's an indication for an intervention okay.
