Is the Air Conditioning Checklist Set Correct?
The board believed that a superfluous green indication on the presurization panel could be easily overlooked when perception was biased by the exception that it should not be present. The first officer for this flight had several comments on his training record that observed that he had difficulty following standard operating procedure and having the correct usage of check lists. He had a tendency to make mistakes and omit items, and tended to over react and lose confidence in non normal situations.
