In the early 1990s, there was a bit of a revolt. People heard rumors that HMOs were telling saying no to things doctors said yes to. And they spread rumors about how terrible H MOs were for saying no tothings doctors would say yes to. They rather have the standard of whatever a doctor says could be useful, I want that. So the fundamental thing would be to get the healthcare providers to actually care about the outcomes. But that's the hard problem.
