How to Determine Whether a Patient With Bilateral High Lower Adenopathy Requires Biopsies

There's always been this long standing debate about whether if you have a patient with asymptomatic bilateral high lower adenopathy, should you biopsy them? Do you want to miss lymphoma? Obviously you don't. And I think there isn't a uniform answer, but the American Thoracic Society basically sort of took a cop out and said it should be made on a case-by-case basis. The risk of bronchoscopy is quite low as it is, but the greatest risk actually comes from pneumothorax.
