How to Fix a Quality System

A lot of rules are put in place because someone broke something or mess something. So it's like, well, we're going to put a rule. So that never happens again. That's lazy root cause analysis. Actually, it's not cause. That doesn't explain why it happened. You need to figure out which process actually allowed it to happen. It may not be a fix to the process where it manifested itself. It might be in a completely different process. Yeah. How else do companies become a little bit more bloated? Well, I think it was what I was just talking about, right? Not wanting to do a real root cause analysis and the easy fixes. Slap
