Dosing Lysics in Renal Failure?

All the diuretics work on the tubule side. They don't work from the blood side except for spirolactone. So as your GFR falls, you need to compensate by going up on the dose. What I use is I use creatinine times 20 to get my lysic dose. It's probably doing BUN plus age for an 80-year-old who's absorbing 10% of it. That's probably why you need to use so much. Maybe it works.
