Do I Need a Lumbar Puncture?

I was just on service and this kept coming up we'd see patients with altered mental status and fever or the question was always do they need a lumbar puncture or not. I find it's almost it's more commonly a rule out than a rule in procedure, Jack says. The tricky ones are well that patient has a little abnormality on chest x-ray and we think they probably have an early pneumonia but gee they're so altered  and should they be this altered from just their UTI? You go looking to make sure and you and you don't find any evidence of meningitis encephalitis.
