The RV and LV Dysfunction in Pulmonary Artery Hypertension

The flattened septum in both diastole and systole suggests we have chronic pressure and volume overload. In most cases of pulmonary arterial hypertension, intrinsic RV contractility is actually not impaired. An exception is an entity like scleroderma where we do seek contractile impairments in addition to increases in afterload. And the latter is also true in patients with heart failure and reduced ejection fraction as well as some forms of heft-peth.
