Introduction
Revolution Airways Flight 8 was a Lockheed L-188C with 15 people on board. The 10 passengers are mostly hunters and anglers, and they're joined by 5 crew members. Shortly after takeoff, the pilots notice an odd vibration. A propeller from engine number 4 detach, go forward from the engine, then veer to the left and strike the fuselage damaging the flight controls. What could have caused this mechanical failure?
