Brietta Clark: Medicaid and Medicare are two of the most underrated civil rights achievements in our history. She says that there's a paradox in the way it's enacted that undermines some of that progress. The states have tremendous discretion in how they design the program, she says. And again, we have to be very careful about who we've seen as "despicable" folks helping their now-retired neighbors.
