What Caused the Door to Burst Open an American Airlines Flight?
The pins were supposed to protrude six point three five millimeters beyond the rear face of the flanges, but instead they were stopped short. There was a mechanism to prevent the handle to be closed when the pins are not aligned. But there was a deformation that allowed the closure. The additional support plate specified in the service bulletin was designed to prevent this deformation, but it was not installed. It showd that the door was locked, even though it was in an unlocked position.
