Surgical Billing - How Do You Document the Details?

Surgical billing is something that you'll probably have done for you by coders, but it's also the most important one to talk to your folder about and learn from. If you come out and say, I just didn't build a linear care and you don't get any details to that, then you may have done a little linear repair for four hours on a more than little piece of patients. How do you document that? Is there a way to prioritize that or something that should be saying?
