The patient who is alert and interactive talking with you, they're probably not terribly poisoned. Go ahead and empirically treat them for any kind of carbon monoxide exposure with 100% oxygen. Then reserve your cyanide treatment for those more critically ill patients. Most places can't test for cyanide, it's a send out. Even the ones that do, it takes about two hours to come back.
