The Future of Passwords
The higher up in the company you get, the less likely you're going to be actually paying attention to password policies. Hardware tokens are fabulous right up to the point that you lose one. If you use it upon logging into every app and then a couple of random times through the day, it becomes seen as something that's just as bad as passwords. There is no consensus yet on what the proper step following passwords should be.
