The Relevance of Symptoms in Acute Coronary Syndrome
There isn't a sign or symptom that is completely diagnostic or rollout and that's why acute coronary syndrome is so difficult. The positive likelihood ratio when someone talks about pain going down both of their arms is only 2.6. A classic was whether or not it improved with nitroglycerin and that essentially is completely useless. It's got a positive likelihood ratio of 1.1. So yeah it's important but it certainly isn't diagnostic and that's an absolute theme. And there are multiple studies which look at how framing and risk factors relate to presentations to ED with chest pain.
