When you pull a large number of patients together and you rigorously control them so the groups are equal, what you end up doing is homogenizing them. And perhaps it should not surprise us to then have a hard time finding interventions that benefit that entire group of people when they're all different. This is one of the common themes at SCCM this year was trying to individualize our understanding of a lot of disease processes.
