In 1958, a B-47E bomber flying over South Carolina accidentally released a nuclear bomb. In 1960, there were signals from the ballistic missile early warning radar that bounced back off the moon as it rose over Norway. Four thermonuclear weapons fell off a B-52 in a mid-air collision over Spain. So these instances were happening all the time. And part of the thing around the use was like, even if, by some miracle, your rational sort of game theory strategy works, there's always just the chance that some bloke drops it off a plane.
