What Is the Cause of the Phantom Limb Pain?

In 2 Kings 526 Elisha says that his spirit was with him when he went to name in to get some silver for himself. That would be consistent with the effects of both an out of body experience, or simply a remote viewing experience. And incidentally I keep a list of events in the Bible where paranormal effects like this happen. And that's on the list, along with a bunch of others.
