The Override Code in Murphy's Death
The override function on the automatic medicine dispenser had not been working as expected. The machine would take forever to dispense medication or wouldn't do so at all. Nurses were instructed just to override the system and manually enter the request for the drug. In fact, at that time, only three per cent of medication requests included in override.
