Tylenol is metabolized to sulfate and glucuronide conjugates. When there's really high levels of acetaminophen in the body like toxic levels, they saturate these safe pathways. And this makes a nasty compound called NAPQI which I'm not going to spell out or anything like that. It's irreversible and it quickly progresses to liver failure. The max limit therapeutic dosing was 80 milligrams per kilogram. Just double that greater than 150 milligram per kilogram is concerning for toxicity.
