This is a very high risk for permanent hypoperathiridism, which would really impede this patient's quality of life. You could potentially only remove the large parathyroid lands. Or you could do a total parathyroidectomy with a forearm transplant. The most fun part of putting the parathyroid tissue back into the arm is that if it comes back later, you can put a blood pressure cuff on that arm and inflate it to isolate the blood there.
