I used to develop middleware services and a lot of them were Windows services. A lot of multi threading, mutexes, semaphores, things like that. And keeping that kind of stuff in line is difficult on a good day. But I can't imagine the scale at which that has to happen with the web... It's got to be an enormous undertaking.
