In the Western world, I feel like evil spirits are as hidden as they were in the Old Testament times. If you do much praying for the sick, you're going to see some demons manifest that you didn't expect to see just in a context of praying for the Sick. And so I think regardless of whether you know, you at least know, tell the demon to go.
