The definitive diagnosis of bacterial peritoneitis is made by tapping the belly and getting an acidic neutrophil count of more than 250. I would never do a tap anymore than using ultrasound, for sure. Have a low threshold for doing it. And just to determine if you want to do a diagnostic of their therapeutic tap, absolutely use ultrasound.
