Dr. Ratan: The approach to the subclavian has always been something that's confused me, especially with the question of what to do with the clavicle. Trapdoor incision connects an enterolateral thoracotomy, sternotomy, and clavicular incision. If your patient is unstable, you don't actually have time to do all this and you should just do a clamshell. But beware that the addition of a trapdoor can be perilous due to collateral if you're not familiar with it.
