Trapdoor incision connects an enterolateral thoracotomy, sternotomy, and clavicular incision. If your patient is unstable, you don't actually have time to do all this and you should just do a clamshell. But beware that the addition of a trapdoor can be perilous due to collateral if you're not familiar with it.
