The Bigger Dose, Wait as Long as You Can
Sean: I'm generally waiting 24 hours. Or I'm encouraging people to wait as long as they possibly can to really try before they start the buprenorphine. So instead of giving two or four milligrams, I am probably telling them to take either eight or 12 right away and ramping it up quickly. Sean: There are times where you just giving 16 for the first dose to, and definitely for people that are using fentanyl or whatever is in the very toxic drug supply at this point.
