How to Stop a Vitamin K Antagonist
If it's a low risk surgery and there are no patient risk factors for example the Hasblet score then you would stop Warfarin. Not every surgery has a known risk so if it's just if it's not known how risky the surgery is or if it's intermediate or high risk then you would stop vitamin K antagonist. In between, use your clinical judgment to decide whether someone should be on Warfarin longer than 5-7 days before they have an operation. If their INR is greater than 3 - this will help identify people who are super therapeutic at the time of the procedure.
