Leaders have a responsibility to model that level of self-critical reflection. The more you model that, the more you will see employees come forward. I spent 17 years in the health care system in leadership rolls. You can create two different cultures around that issue. One is about trying to im tighteng things up and not make mistakes any more. And so then there's no ability to resolve it. Whereas we really just want to say, when we make a mistake, we want you to report it. We want people to be able to look at what contributed to that?
