In 1972, Eastern Airlines 401 crashed outside of Miami International Airport. The pilot and co-pilot didn't know if the landing gear had deployed properly. These were the disasters that spurred the development of CRM. But people have found that it applies to anywhere where you have people working together that can turn into a life or death situation.
