As a PhD student in Harvard, I was invited to study medical errors. And what I found was that this whole business of learning from error, as we know, was quite fraught. But then the number one thing that I had not anticipated, but became quite starkly true, was that there were real palpable differences across teams and how willing they were to talk about error.
