How to Write a Response in a GIRP Form

The P for plan covers exactly the same information in GIRP as it does in the P for plan and DAP. The response section is our opportunity to speak about the client's symptoms and also how they're responding to you in session. So let me give you an example of a great response section from the same soap note. Client was able to identify multiple self care activities and committed to completing at least three of them in the next week.
