How to Ventilate Patients With Chronic BPD With High Tidal Volume

The BPD lungs have a heterogeneous feature as compared to the two compartment models seen in patients with RDS. Some areas of the lungs may have normal compliance and resistance, whereas others may have poor compliance and high resistance. As such, it is important to manage the patient with severe chronic BPD with high tidal volume. This allows for more gas to fill the lungs. Couple this high tidal volume, which is about 8 to 10 ml per kilo with higher eye times and low rates to decrease the risk of at electasis.
