Having a Running Log of All Issues in the Company

YC has started a running log of all issues in the company. Anyone can add to it anonymously or with their name on it. The idea is that like, you I think we really value building a culture where we can be real with each other. It's far better to drag it out into the light where it can be solved than to have it fester quietly.
