Do You Have to Change the First Patent to Renew the Second?

Pharmaceutical companies have to invest millions in researching a new pharmaceutical and technical and clinical trials, because most of them fail. And the compensation that they get for that is the patent period that gives them exclusive period to license it over a period of time. But what about when they create modifications to that original patented work that then? Does that do you get to extend the full patent term?
