Is There Really a Mistake?

The word mistake is really mysterious and how do you even define what that is? Let's say there is a terrible mistake, something was missed or are misdiagnosed. I don't think any of us make excuses for ourself. And my intention is not to suggest that we should give ourselves a pass. That's not possible for most of us just as thinking, feeling sentient human beings.
