The Fear of the Animal Trainer
Special effects supervisor John Richardson was responsible for a decapitation scene in the movie. The next day that animal trainer was killed by tigers. According to Richard Donner, Lee Remick, who played the mom, was legitimately terrified while filming this sequence. So her screams are very real and they're what's in the final shot of the film.
