The idea of learning from decisions is something linking that with a journal. What did I feel at the time? What was I thinking was going to happen? What actually happened? Is there a different way I can think about it is really critical? Bad technique only fails one out of every 10 times if you're having 90% success with your non-difficult approach. This is what happens with bad intubation, which is not going to do the self reinforcement that needs to be done.
