Dr. Ellis: It seems to me a very flawed concept to say that if there is no left atrial appendage, that you still have to anticoagulate the patient for a month after cardioversion. People without appendages go in and out of persistent AFib on their own all the time, and we don't reanticoagulate them, right? There's not anything specific about the conversion from AFib to sinusarhythm that causes left atrial thrombus.
