Pulmonary renal syndrome is most often caused by inflammation and subsequent necrosis of the small tiny vessels in the lungs and in the kidneys. In this case, the positive anca titer clues us in that this patient may have an anca-associated vasculitis. Glomerular pathology can largely be broken down into nephrotic syndrome where large amounts of protein leak into the urine or nephritis in which red blood cells often dysmorphic from having traveled through a broken basement membrane and up in the urine.
