A case of an elderly lady came to us with an altered mental status. Her past medical history was notable for a liver transplant. Transplant recently, and she was on immuno suppressence for it. On physical exam, we found that she had tremors, disartia, pasticityand hyporiphlexia. This really confused us, because it was hard to localize where the pathology was. And soa he on the top of our differential wors, infections and a metabolic cause of her altered mental status were ruled out. When we didth, when we scanned her head, we foundthat she had a hyper intensity in her pons and throughout her central nervous system.
