In a military context, they can engage in an awful lot of training to try to prevent scenarios like that. But how you deal with it afterwards when something goes wrong is a big issue. You can't really sweep it under the carpet. And I don't think you should confront your imperfection in life. It doesn't mean you have nothing to contribute and therefore you should stay quiet.
